Job Description
LOCATION: Lancaster, PA
HOURS: Full-Time, Day Shift – Monday through Friday from 8:00am-4:30pm.
POSITION SUMMARY:
Responsible for the clinical coordination of referrals to various departments within the Neuroscience Service Line ensuring the prompt scheduling of patients that meet Neurosurgery and Pain Criteria.
ESSENTIAL FUNCTIONS: Qualified individuals must have the ability (with or without reasonable accommodation) to perform the following duties:
- Represents Penn Medicine via telephone and functions as a clinical liaison for all referral sources including physicians, hospitals, outpatient and ambulatory care practices.
- Responds to phone calls, electronic transmissions, e-mails and faxes regarding clinical referrals. Returns messages promptly.
- Serves as a Neuroscience representative in communicating information related to Neurosurgery and Pain services, intake policies, and insurance coverage to patients, families, and referrals sources.
- Accurately completes all referral documentation including obtaining and reviewing orders to facilitate the intake process.
- Uses clinical assessment skills to review referrals and identify additional care needs or a more appropriate care setting.
- Develops professional relationships with all referral sources.
- Proactively offers alternatives if Penn Medicine Neuroscience Service line is unable to provide services.
- Processes and documents referrals that are not accepted for admission, indicating specific reason for non-acceptance.
- Provides and supports scheduling and access reporting data to Neuroscience Leadership.
- Manages assigned EMR work queues and follows up with supervisor as needed
- Identifies issues and provides information to supervisors as needed for resolution.
- Presents excellent customer service orientation at all times.
SECONDARY FUNCTIONS: The following duties are considered secondary to the primary duties listed above:
- Participates in the development of policies and procedures and actively assists in problem solving with and between departments to ensure the clinical intake function is collaborative and effective
- Maintains positive professional relationships with support personnel through appropriate and timely responses for information as necessary for clinical and billing functions.
- Identifies trends in referral patterns and participates in quality improvement activities
- Other duties as assigned.
JOB REQUIREMENTS
MINIMUM REQUIRED QUALIFICATIONS:
- Current licensure as a Licensed Practical Nurse (LPN) (or as a Registered Nurse (RN)), issued by the PA State Board of Nursing issued by the Commonwealth of Pennsylvania.
- Must have a minimum of one year of clinical experience.
