Care Connections is searching for a full time physician to see patients in our unique practice serving patients with chronic medical conditions. Primary care providers see patients in both outpatient, inpatient and home care settings. Our holistic approach to patient care allows us to focus on patient well-being and provider satisfaction. Our physicians enjoy a Monday through Friday schedule, dayshift only, with minimal call.
ESSENTIAL FUNCTIONS : Qualified individuals must have the ability (with or without reasonable accommodation) to perform the following duties :
- Provide ambulatory, home, and inpatient care to patients
- Attend to patients enrolled in the program in the hospital on a daily basis in a consultative fashion
- Take after-hours call in rotation with other medical staff to provide 24 / 7 coverage for patients via phone.
- Team based care with nurses, social workers, rehabilitation therapists, pharmacists, psychologists, and other health care providers.
- Consults with patients to determine the appropriate course of treatment.
- Recommends and orders tests to determine extent of illness or help diagnose condition.
- Monitors lab results, medication adherence and medication toxicities.
- Prescribe or administer treatment, therapy, medication, vaccinations, and other medical care to treat or prevent illness, disease, or injury.
- Explain and discuss procedures, test results, or prescribed treatment with the patient.
- Refers patients to medical specialists when necessary.
