Summary:
- Works under the direct supervision of the Registered Nurse and supports both the Registered Nurse and Certified Nursing Assistant by providing continuous observation to patients that require 1:1 observation. Supports team focused relationships to ensure patient safety and improved outcomes. Demonstrates accountability to the patient, family, self and the healthcare team.
Responsibilities:
- Patient/Customer:Identifies patient using 2 patient identifiers according to PPMC policy before carrying out functionsReceives report from the RN and NA including pertinent information related to patient care at change of shiftIntroduces self and ask patient how he/she wishes to be addressedCommunication with patient using interventions to decrease agitation and confusionProtects privacy of the patient and observes confidentiality policyProvides optimal stimulation and appropriate activities based on the patient needsSupports and contributes to Penn Presbyterian Medical Center patient satisfaction goals
- Age Specific:Provide age-appropriate activities and communication
- Patient/Employee Safety:Maintains a safe environment and protects patient from injuryPerforms 1:1 observation according to PPMC policyAccompanies patient when leaving the unit, does not leave the patient unless relieved by another staff membersUtilizes safety devices, places call bell within patients reachPromotes infection control: performs proper hand hygiene, maintains universal precautions, carries out isolation procedures
- Operations:Document 1:1 observation according to PPMC policyEnsure the patient is safe and that assistance is able to be provided in a quick and timely mannerPerform duties in accordance with PPMC mission, values, policies, and proceduresSupport and contribute to PPMC patient satisfaction goalsEstablishes and maintains good working relationshipsPrepares meal tray and patient for mealtime, assists with feeding patients are not on swallowing precautionsAssist RN/CNA with incontinence care, turning the patient, and patient hygieneAssist patients out of bed and ambulate patients if appropriate in room, to bathroom, or hallways.Assist with patient transfers from bed to chair, wheelchair, stretcher. If patient not independent, assist nursing staff with patient’s mobility
- using proper body mechanics.
Credentials:
Education or Equivalent Experience:
- H.S. Diploma/GED (Required)
- Education Specialization: Equivalent Experience:•And Prior Healthcare Experienced Preferred
