HUP is looking for a full-time Social Work Team Leader!
Responsibilities:
Support effective Social Work activities; manage performance of Social Work staff and provide them with continuing educational programs/training; collaborate with the Social Work Director to develop efficiency within the department- Address psychological, social and system issues of complex patients
- Support utilization management activities: communicate with insurers, interdisciplinary care team and Physician Advisor
- Lead discharge planning activities: assure timely coordination of home care, home medical equipment, placements, hospice, community resources, transportation
- Participate in quality data collection and risk management referrals
- The expected weekly caseload of the Social Work team leader is adjusted to ensure achievement of team leader accountabilities
Credentials:
- PA License (LSW or LCSW) required
Education or Equivalent Experience:
- Master’s Degree in Social Work or related field required, two years of clinical experience required with five years of clinical experience preferred.
