SUMMARY OF POSITION:

Hybrid, mostly remote with onsite meetings.

The Population Health Clinical Value Associate is integral in supporting Population Health activities. Population Health strives to enhance the patient experience, improve quality, reduce costs for a patients’ care and improve care transitions. This individual will need to adjust interventions in a manner that is sensitive to different cultural, religious, economic, and social groups and how it affects the knowledge, delivery, and acceptance of healthcare services. This individual is responsible for supporting primary care objectives through targeted outreach to our patients, data mining, surveillance of health maintenance areas, coordination of care with the broader care team at the clinic as well as Pharmacists, Care Managers, and Social Work. This individual will support monitoring of work queues and proactively create relationships with providers and patients to ensure optimum health and wellness for our patient population. The ideal candidate is someone who takes initiative and can work independently, is outgoing and friendly, and builds relationships with ease. A quick-learner with strong critical thinking skills and who has the ability to be flexible will excel in this role.


MINIMUM QUALIFICATION(S):

  • High School Diploma or equivalent required.
  • Must meet current eligibility to sit for the exam with attainment of certification or registration through one of the Penn State Health approved agencies for Medical Assistants within 90-days of employment date or current active status as a Certified Medical Assistant (CMA) or Registered Medical Assistant (RMA) through one of the Penn State Health approved agencies for Medical Assistant certification required.
