SUMMARY OF POSITION:

Responsible for the coordination of follow up health care for a defined patient population. Assists patient in scheduling office visits as needed. Assists RN Care Managers by identify high risk and/or chronic disease patients. Deploys excellent customer service engagement skills by phone.

MINIMUM QUALIFICATION(S):

• High School Diploma or equivalent required.
• Registered or Certified Medical Assistant required.
• Three (3) years of Health Care experience working within an ambulatory practice or population required.
Graduate of an accredited Medical Assisting Program or Licensed Practical Nurse License required.

PREFERRED QUALIFICATION(S):

• Previous use/exposure to using an EHR/EMR, working with clinical data, clinical reporting and other analytics tools preferred.
