What are the responsibilities and job description for the Adult Mental Health Case Manager position at PennReach, Inc?
PURPOSE: To use best practices and technology within a community-based setting to teach people with severe and persistent mental illness, how to acquire the physical, intellectual, emotional, and social skills needed to live as independently as possible.
QUALIFICATIONS:
· Bachelor's level community support staff who shall have graduated from an accredited college or university with a bachelor's degree in one of the helping professions, such as social work, human services, counseling, psychiatric rehabilitation, psychology, or criminal justice. Staff with a bachelor's level degree in a field other than a helping profession shall have a minimum of two years of professional experience in a community behavioral healthcare setting. Certification as a psychiatric rehabilitation practitioner may be substituted for one year of experience.
· A Master’s degree from an accredited college or university in one of the helping professions, such as social work, human services, counseling, psychiatric rehabilitation, psychology, or criminal justice.
· At least one year of experience in a mental health setting.
· A valid driver's license in the State of NJ with good driving record.
· Shall be at least 18 years of age or older.
· Employee must cooperate with the licensee and Department staff in any inspection or investigation.
RESPONSIBILITIES INCLUDE:
- A 40-hour salaried, flexible schedule based on the needs of the individuals supported.
- Must successfully complete and demonstrate proficiency in all areas of required training.
- "On call" 24 hours a day in cooperation with other management staff.
- Monitors and ensures that residential budget is maintained.
- Monitors and schedules required staff training, as well as relevant seminars, conferences, and additional training.
- Monitors and insures accurate documentation.
- Monitor apartment, office, and vehicles maintenance.
- Ensures the accuracy of the disbursement and documentation of individual finances, as needed.
- Monitor health and medical needs of individuals and audits Medication Administration in conjunction with nursing staff, if needed.
- Builds a support network between the individuals, community members, and natural supports.
- Monitors the documentation of the individual’s progress for support people including family, staff, employment specialist, case manager and other professionals, as necessary. Documents observations, information and services provided to the consumer resident in progress notes.
- Attend conferences, seminars, and training relevant to the position.
- Communicating observations and information about the consumer to the program coordinator and support team.
- Supervise, organizes, and supports recreational and/or socialization activities.
- Provide transportation to consumer resident in a company car as needed.
- Provide residential supports and education.
- Takes the lead in individual service coordination activities as identified.
INITIATIVE REQUIRED:
Fulfillment of the job responsibilities takes place in the residence, agency, and in the community. Work is reviewed by the supervisor according to the policies of PennReach.This review includes such things as random site visits, systematic assessments, and employee evaluations. The Case Manager must be able to handle the pressure of engage with consumers in a public environment, exercise patience, work with flexible schedules, and respond to crisis situations. The ability to effectively communicate with staff, other agencies and members of the community is crucial to successful integration.
The position also requires the full use of good judgment and common sense during daily affairs. Professional communication and appropriate language, whether written or oral, is essential in everyday activities. This communication may be with any person involved in the life of the individuals living in the home. Ingenuity and creativity must be utilized when determining the best training methods, assistance and supports for an individual.
