Job Overview
We are seeking a dedicated and compassionate Case Manager to join our foster care team. The ideal candidate will play a crucial role in supporting individuals and families by providing guidance, resources, and advocacy. This position requires a strong understanding of social services, crisis intervention, and behavioral health to effectively assist clients in navigating their challenges. A foster care case manager is responsible for overseeing the well-being of children placed in foster care, including assessing their needs, coordinating placements with foster families, providing ongoing support to both children and foster parents, working with biological parents towards reunification when possible and advocating for the child's best interests throughout the foster care process, all while maintaining detailed case documentation and adhering to child welfare regulations.
Responsibilities
- Conduct comprehensive assessments to determine client needs and develop individualized service plans.
- Provide ongoing support and counseling to clients, including crisis intervention when necessary.
- Collaborate with healthcare providers, social workers, and community organizations to coordinate services.
- Monitor client progress and adjust service plans as needed to ensure effective support.
- Advocate for clients’ rights and access to necessary resources such as childcare, housing assistance (including Section 8), and workforce development programs.
- Facilitate referrals to appropriate services including behavioral health resources and child welfare programs.
- Maintain accurate documentation of client interactions, progress notes, and case files in compliance with agency policies.
- Participate in training and professional development opportunities to enhance skills in mentoring and crisis management.
Requirements
- Bachelor’s degree in Social Work, Psychology, or a related field preferred; relevant experience may be considered in lieu of degree.
- Experience in social work, childcare, or related fields is highly desirable.
- Knowledge of Medicare processes and regulations is a plus.
- Strong crisis intervention skills with the ability to manage high-stress situations effectively.
- Familiarity with behavioral health issues and community resources available for clients.
- Excellent communication skills, both verbal and written, with the ability to build rapport with diverse populations.
- Strong organizational skills with attention to detail for maintaining accurate records.
- Ability to work independently as well as part of a multidisciplinary team.
