Key Responsibilities
- Assess the psychosocial status of patients concerning their illness and environment, and effectively relay findings to the registered nurse.
- Provide essential information and support to patients, families or caregivers, and community agencies.
- Assist physicians and team members in understanding the critical social and emotional factors that pertain to health issues.
- Foster collaborative relationships with organizational personnel to enhance patient care outcomes.
- Establish and maintain contracts with public and private agencies to serve as resources for patients and staff.
- Proficient in social work assessment methodologies.
- Strong collaborative abilities to work with healthcare professionals.
- Excellent communication and interpersonal skills.
- Problem-solving aptitude related to patient care.
- Familiarity with community resources and referral services.
- Graduate of a master’s program in social work accredited by the Council on Social Work Education.
- A minimum of one (1) year of experience in the healthcare sector is essential.
