Key Responsibilities
- Direct and administer physical therapy treatment in patients’ homes, while thoroughly assessing the home environment.
- Identify priority needs for physical therapy interventions.
- Communicate patient reactions to treatments and any changes in conditions to the attending physician.
- Recognize the needs of patients and their families/caregivers for additional home health services, providing referrals as necessary.
- Engage in discharge planning for patients.
- Supervise physical therapy assistants in accordance with organizational policies and state regulations.
- A degree from an accredited baccalaureate or master’s program in physical therapy.
- Valid licensure to practice as a physical therapist in the state, as applicable.
- A minimum of two years of relevant experience as a physical therapist, with a preference for candidates with community or home health experience.
- Current CPR certification.
- A valid driver’s license and reliable transportation.
