Pennsylvania Hospital is looking for a Parent / Baby focused Social Worker!

Summary :

Responsible for psychosocial support on selected patient populations and collaborates with the unit / service CM / SW team for discharge planning and contributing to the patient's treatment plan through and understanding of social, psychological and / or environmental needs related to the impact of illness and need for hospitalization. Professional activities will be carried out in accordance with the code of ethics established by the National Association of Social Workers.

Responsibilities :

Complete comprehensive biopsychosocial assessments for parent on postpartum floor and infant in ICN.

Provide resources and referrals to hospital-based and community programs

Arrange home care, DME and other discharge resources as needed

Report to and collaborate with Children & Youth agencies when necessary to determine safe discharge planning

Knowledge of perinatal mood disorder, trauma informed care and able to provide short-term clinical interventions

Schedule and attend family meetings

Engage in palliative care / goals of care discussions

Provide grief support in response to infant deaths

Coordinates and collaborates with the unit / service CM / SW staff around the discharge planning process to ensure a safe and timely transition from hospital to home, community, or institutional setting around follow-up community behavioral health services.

Collaborates with the behavioral health interdisciplinary health care team to ensure continuity of care and support services to patients and families. This includes a AP and psychiatrist assigned to the MEND program.

Identifies and attends to the psychological needs of patients and families.

Provides counseling to patients, families and groups.

Responsible for documentation in the medical record, statistics and reports as required.

Maintains relationships with community agencies and keeps up-to-date on resources.

Uses effective interpersonal skills in all interactions.

Enhances own professional development and maintains formal and informal continuing education for self-improvement.

Additional Requirements / Preferred Experience and Skills :

Hospital SW experience, preferably with children / families or NICU

Able to work independently in a fast-paced environment

Able to collaborate with multidisciplinary teams across Parent / Baby and ICN service lines

Experience with providing support during infant mortalities preferred

EPIC proficient

Credentials :

Licensed Social Worker

MSW or LSW preferred.

Education or Equivalent Experience :

Master of Arts or Science (Required)

Education Specialization : Social Work Equivalent Experience :

And 3 years Behavioral health experience in the field, health care setting experience helpful, direct practice experience helpful.
