About the Role
Penny Lane Centers' Transitional Housing (TH) Program seeks a highly motivated and compassionate individual to serve as a Case Manager. The successful candidate will work closely with Transition Age Youth (TAY) between 18 to 24 years of age, who are homeless or at risk of being homeless.
As a Case Manager, you will facilitate a collaborative process of assessment, planning, facilitation, care, coordination, evaluation, and advocacy for options and services to meet an individual's comprehensive needs through communication and available resources to promote safety, quality of care, and cost-effective outcomes.
