What You’ll Do:
As our Social Services Director, you will:
- Welcome and orient new residents and families to the facility, providing support during the adjustment period.
- Conduct interviews to gather social histories and complete assessments, including MDS documentation.
- Develop and implement personalized social care plans, addressing residents' emotional and social needs.
- Collaborate with interdisciplinary teams to conduct resident care conferences.
- Advocate for residents’ rights and ensure their concerns are heard and resolved.
- Manage the discharge process, coordinating with external providers to ensure smooth transitions.
- Maintain accurate and compliant social service progress notes and documentation.
- Refer residents and families to appropriate community resources when needed.
- Stay updated on state and federal regulations governing social services in long-term care facilities.
What You Bring:
- A Bachelor’s degree in Social Work or a related field (required).
- At least 2 years of experience in a skilled nursing or long-term care facility (required).
- Knowledge of social services practices, laws, and regulations in healthcare.
- Strong organizational and leadership skills with a compassionate approach.
- Ability to work collaboratively in a team setting while managing multiple tasks effectively.
- Good physical and mental health to perform the role.
