What are the responsibilities and job description for the In-Reach Peer Support position at Pennyroyal Center?
This employee will assist in providing services to adult clients meeting diagnostic criteria of Severe Mental Illness to ensure these clients receive necessary services to maintain stability of their SMI symptoms and with the goal of avoiding/reducing hospitalizations and preserving their appropriate placement in the community.
Job Responsibilities include:
1. Must completed Kentucky approved Adult Peer Support Specialist (APSS) Certification training/pass exam within 6 months of hire. To participate in ongoing Supervision according 908 KAR 2:220 Adult Peer Support regulations (Kentucky).
2. Must be able to establish rapport with clients, families, and cooperating entities, engaging clients through outreach and support.
3. Using personal experience of recovery to assist clients.
4. Advocate to and for clients by sharing first-hand experiences, offering hope and reassurance.
5. Provide interventions to clients regarding (but not limited to) symptoms of mental illness, coping skills and resources, medication benefits and side effects, relapse prevention.
6. Assist clients in developing self-directed care and advance directive plans.
7. Promote recovery through role-modeling, sharing personal experiences, connection to community, self-determination.
8. Complete all documentation timely and accurately in the format approved by funding sources and regulatory statutes.
9. Act as liaison between client and community; building relationships with families and cooperating entities to develop additional resources and coordinate services.
10. Must have transportation and be willing to transport clients if needed.
11. Other duties as required/assigned by supervisors.
Requirements for this position include:
Minimum of High School diploma, one year experience in MH services preferred. Must pass required training within specified timeframe. Must have current driver's license, proof of automobile insurance, reliable transportation, and be able to travel throughout the region. Must present proof of a recent (less than 1-year-old) TB skin test, and must pass a legally required Criminal Record Check and Sexual Offender Registry check.
