REGIONAL TRANSITIONAL
CASE MANAGER, SMI
REPORTING/COORDINATING RELATIONSHIP: Coordinator of CSP/SMI Case Management
SUPERVISORY RESPONSIBILITIES: None
PRIMARY RESPONSIBILITIES: This employee will provide case management services to adults with a diagnosis of a severe mental illness throughout the Pennyroyal Region.
Duties include:
- Provide intensive case management to identified clients until they can be placed with a long-term SMI case manager.
- Will maintain a caseload of clients in addition to those clients in transition.
(3) Have ongoing contact with community agencies necessary to develop additional resources.
(4) Assist in developing and accessing housing for clients.
(5) Must be able to establish rapport with clients and families.
(6) Must have transportation and be willing to assist clients by providing outings and keeping scheduled appointments with various community agencies throughout the Pennyroyal region.
(7) Employee is also responsible for timely record keeping associated with the services provided.
(8) May fill in from time to time during the absence of other staff, as assigned by supervisor.
(9) Attend one (1) individual and one (1) group supervision monthly as scheduled by supervisor.
(10) Complete and maintain documentation of a minimum of six (6) hours of training or continuing education each year.
(11) Other duties as assigned/required.
MINIMUM QUALIFICATIONS:
Employee is required to have at least a bachelor's degree in psychology, social work, or related field with at least 1 year of experience in working in a human services setting. Must have a valid driver's license, reliable transportation, and be willing to transport clients on a daily basis in any of the counties in the Pennyroyal region. Must present proof of automobile insurance and pass a Criminal Record Check. Employee must present proof of a recent (less than 1-year-old) TB skin test. Within 6 months of employment, this employee must attend the Core and SMI case management trainings.

Pay: $15.00 - $16.00 per hour
