Are you a collaborative individual who is passionate about ending homelessness in our community? PCHC’s Hope House Health & Living Center is looking for a Transitional Housing Navigator! In this dynamic role, you’ll support individuals residing in transitional housing by providing compassionate support, ensuring safe and sanitary living conditions, and connecting tenants with essential services. You’ll foster a supportive environment where residents are empowered to achieve greater stability and independence. Schedule : Per-diem, flexible schedule could include nights and weekends (NOTE : Shift differential available for some shifts)

Highlights of the position :

  • Engages directly with individuals seeking residential housing services, fostering a supportive community atmosphere and treating all humans with kindness, dignity, and respect.
  • Is a good steward of confidentiality and discretion. Collects, completes, processes, and files all applicable intake documents for tenants, maintaining a thorough understanding of all necessary documentation.
  • Assists tenants with reasonable requests and connects them to services and resources as appropriate.
  • Provides consistent oversight of tenants, addressing behavioral and / or safety concerns in alignment with policies. Responds to and manages crisis situations with established protocols, prioritizing tenant safety and well-being.
  • Performs basic housekeeping and additional support functions, including but not limited to :
  • Preparing rooms for new tenants
  • Ensuring shared spaces are clean, sanitary, and well-maintained.
  • Distributing meals to tenants, ensuring timely service and equitable access.
  • Collaborates to ensure services are mission-focused. Continuously looks to seek improvement and implement best practices.

  • Education and Experience :

  • High school graduate or equivalent preferred
  • Experience and / or education in Mental Health or Chemical Dependency is preferred.
  • Will have up-to-date training and certification in Healthcare Provider Basic Life Support issued by American Heart Association, American Red Cross or American Health & Safety Institute. Must obtain within 2 weeks if not current at hire.
