Do you have a passion for supporting individuals with developmental disabilities? People Creating Success, Inc. is seeking a dedicated and compassionate Community Based Support Coordinator to join our Ventura County team at our Oxnard office!
In this role, you will play a vital part in overseeing all aspects of our community-based day services, ensuring the well-being and growth of the individuals we support.
What Will You Do?
- Develop, monitor, and revise Individual Program Plans (IPPs)
- Advocate for individuals' needs and connect them with essential resources
- Provide direct support and guidance to individuals, fostering their independence and personal development
- Collaborate with families, conservators, and Regional Center representatives to ensure comprehensive care
- Conduct trainings, troubleshoot challenges, and maintain a safe and supportive environment
- Uphold agency policies and demonstrate a caring and positive attitude in all interactions

What Do You Need?
- Bachelor's degree from an accredited college/university (Master's degree preferred)
- Minimum four years of experience working with individuals with developmental disabilities
- Minimum two years of supervisory experience
- Two years of experience in Supported Living and CCL programs
- Strong knowledge of Person-Centered Thinking and relevant regulations
- Excellent communication, problem-solving, and organizational skills
- Valid driver's license, clean driving record, and ability to travel throughout California
