Do you have a passion for supporting individuals with developmental disabilities? People Creating Success, Inc. is seeking a dedicated and compassionate Community Based Support Coordinator to join our team!
In this role, you will play a vital part in overseeing all aspects of our community-based day services, ensuring the well-being and growth of the individuals we support.
What Will You Do?
Develop, monitor, and revise Individual Program Plans (IPPs)
Advocate for individuals' needs and connect them with essential resources
Provide direct support and guidance to individuals, fostering their independence and personal development
Collaborate with families, conservators, and Regional Center representatives to ensure comprehensive care
Conduct trainings, troubleshoot challenges, and maintain a safe and supportive environment
Uphold agency policies and demonstrate a caring and positive attitude in all interactions

What Do You Need?
Bachelor's degree from an accredited college / university (Master's degree preferred)
Minimum four years of experience working with individuals with developmental disabilities
Minimum two years of supervisory experience
Two years of experience in Supported Living and CCL programs
Strong knowledge of Person-Centered Thinking and relevant regulations
Excellent communication, problem-solving, and organizational skills
Valid driver's license, clean driving record, and ability to travel throughout California (if applicable to the position)
