Job Summary:
We are seeking a dedicated and compassionate individual to provide instruction and supervision to people participating in our Community Services Programs. The successful candidate will work with individuals in their own homes and in the community to increase their independence in activities of daily living.
Essential Functions/Responsibilities
Assist individuals in skill-building activities as defined by the Staff Action Plan, which may include:
- Gaining and maintaining independence in areas of daily living skills
- Providing a break to caregivers (In Home Respite only)
- Accessing and/or being involved in the community
- Learning social skills while at home and/or in the community
- Other duties as outlined in the Staff Action Plan and assigned tasks
Participate in general staff meetings, in-service training, and other relevant agency meetings
Assist program participants during scheduled hours, including days, evenings, and weekends
Travel to multiple locations throughout the agency to provide services as required
Safely transport individuals to and from activities and appointments using a personal vehicle
Submit necessary billing and program documentation, ensuring EVV compliance
Comply with all applicable regulations, policies, and procedures, as well as agency-required trainings
Minimum Qualifications
- 18 years of age
- High School Diploma or GED
- Valid Driver's License meeting agency policy requirements
- Able to transport individuals in a personal vehicle that meets agency standards
- Proof of valid vehicle insurance
- Able to use a personal device (cell phone, tablet, etc.) for documentation
- Able to meet physical demands of the position, including standing, walking, sitting, bending, squatting, kneeling, and climbing
