Description
Northside Community Support Program-ARMHS Practitioner
People Incorporated Mental Health Services is seeking a ARMHS Mental Health Practitioner to join our integrated wellness services team at our Northside Community support program. Northside Community support is a community support program offering wellness services to Hennepin County communities that are underserved. We are expanding the program into a wellness hub providing access and support for recovery. We provide access to various community-based services such as recreational activities, support for job seekers, housing support, therapy, occupational therapy, Art, ARMHS services, and community events.
Schedule : Full-Time, Monday - Friday 8 : 00am-4 : 30pm
Location : Northside Community Support : 1309 Girard Ave N, Minneapolis MN 55411
Hiring Range : $22.00 - $26.00 Hourly, plus $1,000 hiring bonus for external candidates

Main Job Duties :
- Using and modeling person-centered principles, actively engage clients in their mental health recovery, and develop and maintain appropriate relationships that support the client in meeting their individual goals.
- Demonstrate intermediate level of clinical knowledge and modeling as it relates to the individual's goals, treatment plan and diagnosis.
- Facilitate and conduct individual services that address a variety of needs and health concerns for the client. May facilitate and conduct group interventions, if applicable.
- Provide and model crisis intervention and de-escalation for clients experiencing mental health symptoms or other emotional and / or behavioral responses while prioritizing client and program safety.
- Facilitate and conduct individual services that address a variety of needs and health concerns for the client. Facilitate and conduct group interventions, if applicable.
- Document interactions and interventions with clients, according to program requirements, into the organization's Electronic Health Record (EHR) and / or record-keeping system.
- Coordination of services through the recognition of the multiple needs of the client, with other providers in the community, in an effort to provide necessary resources to the client.
- Provide various skill-building, teaching and / or coaching as it relates to the individual.
