Duties Description The Habilitation Specialist 1 (HS1) has unique responsibilities to provide for each individual on their caseload at Noxon Day Hab., and is needed to ensure continuation of day habilitation services for all individuals participating in their assigned program and is essential for providing opportunities for socialization These two HS1s will be responsible for reviewing and acknowledging life plans. This position has a current caseload of 15 with a capacity for 25.Duties include, but are not limited to: • Oversees individual data and maintains all the files in the Day Hab which includes the Welcome book, Res Hab books and uploading files into the EHR.• Write, oversee implementation, and ensure data collection for Safeguard Summary Plans and Staff Action Plans.• Train all IRA staff in implementation of SSP's and SAP’s, data collection, and continuing notes to substantiate Res-Hab billing. • Rectify Res-Hab billing issues with RSFO.• Provide and/or ensure training of all IRA staff as needed in response to DQI audits, regulations, and their roles in Medicaid Compliance. • Write, revise as needed and oversee implementation of individuals” annual and semiannual Plans. • Follow up training and supervising direct care staff in the areas of individual care to ensure standards of care. -Participate in DQI and Medicaid Compliance audits, implement and oversee implementation of corrective actions as needed under the oversight of TTL. • Complete HCBS Satisfaction Surveys for all individuals on the caseload on an annual basis.• Other duties as assigned by supervisor.https://www.linkedin.com/company/nys-office-for-people-with-developmental-disabilities/posts/?feedView=all #LI-DL2
