What are the responsibilities and job description for the ADMISSIONS SPECIALIST position at PeopleInc?
Hourly Pay Rate: $20-$21.85
Responsible for completing admissions process; organizing and maintaining the collection and storage of clinical information related to individuals requesting services. Obtains necessary documents for service requested. Responsible for processing DDP-1s to ensure appropriate enrollment. Keeps information updated in People Inc.’s systems for utilization and support to other departments. Maintains a comprehensive understanding of the services the agency provides.
ESSENTIAL FUNCTIONS/RESPONSIBILITIES:
- Completes admissions process with all individuals interested in obtaining services at People Inc.
- Facilitates program enrollment, coordinating with Care Managers and families to secure required documents.
- Works closely with agency departments to maintain communication, support, and knowledge of services.
- Tracks new enrollments to ensure all required agency paperwork is obtained. Provides follow-up on any outstanding documentation.
- Responsible for adding information to the appropriate database for applicants.
- Monitors OnBase for DDP-1 changes and uploads information to CHOICES utilizing OnBase. Ensures DDP-1s are processed in CHOICES, provides follow up as appropriate and maintains tracking of process.
- Enrolls and updates Individual Data in applicable databases for all applicants based on department requirements, including enrollments into People Inc., changes in program status and residential moves. Does proper notifications to other departments.
- Supports Family Education and Training (FET) or other programs to ensure participation, including, but not limited to enrollment paperwork, scheduling, participation and maintaining files as needed and ensures all documentation is Medicaid Compliant.
- Attends Department and Program staff meetings.
- Participates in Agency Intake Committee.
- Participates in outreach and community events on behalf of People Inc.; these may occur outside of normal business hours.
- Competes People Inc. required trainings, attends trainings as required by OPWDD/DOH and those necessary to fulfill job requirements.
- Maintains confidentiality; handles sensitive information in accordance with agency and departmental policies.
- Completes weekly schedules accurately and timely in accordance with department policy.
- Complies with all agency policies and procedures.
- Other duties as assigned.
MINIMUM QUALIFICATIONS:
- Bachelor’s Degree with 1year relevant experience.
- Working knowledge of developmental disabilities, special needs and awareness of community services and programs.
- Ability to organize, plan and manage caseloads.
- Must have sound judgment, flexibility, dependability and good writing skills.
- Ability to handle multiple tasks simultaneously.
- Ability to travel to various locations located in but not limited to Erie, Niagara, Chautauqua, and Monroe County.
- Ability to meet physical requirements of the position when performing the following: standing, walking, sitting, bending, stooping, squatting, kneeling and climbing
