What are the responsibilities and job description for the DIRECT SUPPORT PROFESSIONAL - AGENCY SUPPORTED position at PeopleInc?

Provides instruction and supervision to people participating in the Community Services Programs, including Community Habilitation and In Home Respite. Works with people in their own homes and in the community to increase their independence in activities of daily living.
ESSENTIAL FUNCTIONS/RESPONSIBILITIES
• Assists the person in skill-building activities as defined by the Staff Action Plan, such as, but not limited to:
o Gaining and maintaining independence in areas of their daily living skills
o Providing a break to the Caregiver (In Home Respite only)
o Accessing and/or being involved in their community
o Learning social skills while at home and/or in community
o Other duties as outlined in the Staff Action Plan and as assigned.
• Participates in general staff meetings, in-service training and other pertinent agency meetings.
• Assists program participants during scheduled hours which may include days, evenings and weekends.
• Safely transports individuals to and from activities and appointments, in their own vehicle. (Com Hab only)
• In all cases responsible to submit necessary billing and program documentation and responsible for any improper or fraudulent submissions. Services provided must be EVV compliant.
• In all cases complies with all applicable regulations, policies and procedures and agency required trainings.
• Other duties as assigned.
MINIMUM QUALIFICATIONS
- 18 years of age.
- High School Diploma or GED perferred.
- Valid Driver’s License that meets agency policy.
- Ability to transport individuals in personal vehicle that meets agency requirements. (Com Hab only, optional for In Home Respite).
- Ability to meet physical requirements of the position when performing the following: standing, walking, sitting, bending, squatting, kneeling and climbing.
- Ability to use a personal device (cell phone, tablet, etc.) that meets agency requirements to complete documentation.
