What are the responsibilities and job description for the SOCIAL WORKER LMSW or LCSW position at PeopleInc?
Salary Range (Annualized): $62,400 to $66,560
Provides clinical social work services to the patients of People Inc. Rehabilitative Health Services and Social Work for the purpose of preventing, assessing, treating, improving, and resolving psychosocial dysfunction with the goal of maintaining and enhancing the mental, emotional, behavioral, and social functioning and well-being of individuals, couples, and families.
ESSENTIAL FUNCTIONS/RESPONSIBILITIES
• Conducts psycho-social assessments (to include diagnosis) and completes assessment based treatment plan, clinical progress notes, reviews and social work summaries, as indicated.
• Develops, carries out, and makes revisions as needed to the treatment plan.
• Collaborates with family/guardians/significant others in understanding the significant social and emotional factors related to the mental health problems.
• Utilizes community resources for linkage and referrals, as indicated.
• Participates in discharge planning, and follow-up services as appropriate.
• Participates in in-service programs and serves as a consultant to other Agency personnel, as appropriate.
• Informs all social work clients of the availability of crisis services/management during non-business hours in the event of an emergency.
• Complies with all policies and procedures.
• Completes other duties as assigned.
MINIMUM QUALIFICATIONS
Graduate Degree in Social Work (MSW)
New York State License
Certification as LCSW preferred
If holding a LMSW licensure, active pursuit of LCSW licensure is required
Minimum of 6 years of experience
Experience with delivering clinical services, behavior management, families, children and developmental disabilities preferred
SUPERVISORY RESPONSIBLITIES
• This position requires no supervisory responsibilities
• This position offers opportunities for career advancement in administrative, and/or supervisor responsibilities
