Reports to : Clinical Supervisor

Job Summary / Objective :

The Community Health Worker (CHW) is a frontline advocate for PCHC patients with a close understanding of the community served. This understanding and / or close relationship enables the CHW to serve as a link between health and social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery. Performs all defined services and other related duties in accordance with the mission of Peoples Community Health Clinic (PCHC).

Protected Health Information Requirements / Access :

  • This position will require the use or disclosure of protected health Information.
  • This position will use the Payment class of protected health information.
  • Restrictions on the protected health information for this position will follow the Privacy Policies of Peoples Community Health Clinic, Inc.
  • Use or disclosure of protected health information not routinely available to this position will follow procedures assessed by or directed by management

Essential Functions :

  • Educates individuals and communities about how to use health and social services systems (including understanding how systems operate).
  • Educates individuals about community perspectives and cultural norms.
  • Build health literacy program(s) and cross-cultural communication(s).
  • Participates in care coordination of services, provides referrals and follow-up.
  • Facilitates social determinants of health services and addresses barriers to resources.
  • Facilitates communication and coordination of services between community partners, PCHC providers and clinical team.
  • Documents all consumer, patient and community encounters in EPIC and maintain confidentiality and accurate reporting.
  • Inform patients, consumers, and community partners about PCHC services.
  • As directed by primary care team, provide coaching and social support encouraging patients / consumers to obtain care or other services as needed.
  • Continuously expands knowledge of changes in community resources.
  • Assist patients and consumers to schedule appointments with Outreach and Enrollment Coordinator to determine eligibility for health care coverage.
  • Conducts outreach through community referral, community events and follow up services.
  • Provides community presentations and in-services to community-based organizations as needed.
  • Supports clinic compliance with all applicable federal, state, local and PCHC rules, regulations, protocols and procedures governing the clinical provision of medical services as well as those relating to, but not limited to, workplace safety, public health and confidentiality.
  • Supports a service-oriented atmosphere in accordance with Peoples Community Health Clinic Mission and Philosophy.
  • Develops and maintains own competence.

  • Ability to work with diverse groups of people and with a large degree of independence
  • Ability to handle multiple tasks simultaneously and to deal with difficult situations
  • Ability to work with and through an interpreter
  • Working knowledge of MS Office Products (ex. Excel, Word, Access, Power Point) and Windows Platform. Must be comfortable moving within an electronic health record regularly

    This position operates in a professional office environment and is not required to wear PPE to perform the essential functions of the position.

    Physical Demands :

    This position requires frequent alternate sitting and standing (34% - 66% of shift); occasional lifting up to 10lbs and carrying up to 20 lbs. (1% - 33% of shift); frequent finger dexterity and use of hands / arms for repetitive movement (34% - 66% of shift); occasional driving, requires 20 / 40 near and far vision.

    Position Type / Expected Hours of Work :

    This is a full-time position. Typically work hours and days are between Monday and Friday,8 : 00 a.m. to 5 : 00 p.m. and some flexibility in hours is allowed. Responsibilities occasionally may require irregular hours as deemed necessary. This may include an adjusted work schedule, long days, and early morning or late evening hours in order to meet client scheduling demands. The employee must work a minimum of 40.0 hours each week to maintain full-time status.

    Travel :

    Frequent travel and driving are expected for this position.

    Education and Experience :

  • Minimum High School diploma or equivalent
  • Bilingual in English and another language preferred
  • Minimum of 1 year of experience in related field
  • Ability to communicate within and across departments and community at large
  • Valid driver license and auto insurance
  • Licensure / Certifications :

  • Community Health Worker (CHW) certification required, or the candidate must be willing to enroll in the CHW Registered Apprentice program completing all components of the program to obtain the national credential within one year.
  • Successful completion of language assessment testing in the primary language of the community served.
