Job Description
We are seeking a skilled Physical Therapist to join our team at Peoples Home Health, LLC. As a Physical Therapist, you will be responsible for providing medically prescribed physical therapy to patients in the comfort of their own homes.
Key Responsibilities:
- Develop and implement individualized plans of care to help patients achieve their goals.
- Conduct thorough evaluations to assess patient needs and create effective treatment plans.
- Provide supervision as needed to PTAs per JCAHO/CHAP, Florida State, and Federal practice act guidelines.
- Maintain accurate records of patient progress and communication with healthcare teams.
You will work closely with our interdisciplinary team to ensure seamless care coordination and provide education to patients and families on proper treatment procedures and supportive activities.
Requirements:
- Certification as a Physical Therapist in the state of Florida.
- CPR certification.
- A valid driver's license and reliable transportation.
