Job Description Summary
Within the community setting, the PHN systematically collects relevant assessment data and identifies specific nursing actions to achieve desired outcomes for individuals, families, and groups. Registered Professional Nurse (RN) license from the State of Illinois. Requires BSN, or RN with a minimum of three years’ experience in population-focused practice. Must complete certification from IDPH for STD Employee Development Guide, HIV Counseling and Testing, and HIV/STD client-centered prevention counseling workshops within one year of employment. Must possess excellent clinical judgment and critical thinking skills. Must possess excellent oral and written communications skills. Knowledge and ability to utilize computers and related technology efficiently, including but not limited to: Windows-based operating systems; Microsoft Office software such as Word, Outlook, and Excel; Presentation software such as PowerPoint and Canva. Familiarity with collaboration software such as SharePoint preferred. Must have valid Illinois Drivers License, proof of auto accident liability insurance and access to reliable transportation (Lead assignment only). Maintains l CPR and AED certification. Must participate in annual training on blood-borne pathogens compliance standards. Ability to maintain confidentiality.
