Purpose of Position:
Position works in the Comprehensive Community Services Program (CCS) which is an entry level community behavioral health services position built on recovery concepts. Under supervision, the employee assists consumers with environmental and supportive services designed to address and overcome financial, personal health or family issues; conducts individual or family assessments to determine service needs; provides agency services when appropriate; and refers consumers for other community-based services when needed. Services to consumers may take the form of protective actions on their behalf or on behalf of the community. Position also provides crisis program services and serves individuals under Chapter 51 mental health commitment orders.
Primary Assigned Program Area:
Mental Health and Substance Abuse and Crisis Program
Duties and Responsibilities:
· Position fulfills requirements of Service Facilitator as outlined in Wisconsin Administrative Code DHS 36.
· Provides community services per Wisconsin statutes, court policies, and/or requirements of program, state, or federal guidelines.
· Provides services in support of recovery concepts and practices.
· Accepts requests and referrals for agency services.
· In collaboration with the consumer and his/her supports, conducts comprehensive, strength-based assessments through interviews, home visits and collecting of collateral information.
· In collaboration with the consumer and his/her supports, determines the range and type of services needed for preventative or rehabilitative services.
· Explains the scope of services and discusses consumer’s rights and responsibilities in relation to the use of services.
· Arranges or provides for appropriate services for consumers based on their assessed needs.
· In collaboration with the consumer, his/her supports, and other providers, assess ongoing consumer needs while developing and updating service plans to address needs.
· Maintains documentation that meets professional practice, billing expectations, and program standards.
· Prepares correspondence, reports, and records as necessary and appropriate.
· Coordinates and participates in recovery teams with the goal of bringing people together to collaboratively work with and for consumers.
· Coordinates and monitors ongoing delivery of services to consumers.
· Participates in committees and task forces as assigned.
· Testifies in court as necessary or required.
· Provides and complies with all crisis program services requirements.
· Provides services to individuals referred to the department under Chapter 51.
· Collaborates with Corporation Counsel to meet court requirements and processes for individuals under Chapter 51 commitment orders.
· Adheres to approved social worker principles, methods and practices, including the National Association of Social Workers (NASW) code of ethics.
· Provides Access duties on a rotating basis.
· Provides intake duties after hours on a rotating basis in compliance with 48.067 and 938.067.
· Provides mental health mobile crisis assessments after hours on a rotating basis.
· Exercises powers and performs intake duties under WI Statutes 48.067 and 938.067, to include taking juveniles and children into custody under WI Statute 48.19, 48.193, 48.195 and 938.19, 938.355 (6d).

Physical Demands:
Primarily sedentary position includes but not limited to: sitting, walking, bending, stooping, minimal lifting, and ability to hear, talk and ability to travel to various offices within courthouse and/or outside of building, including consumer’s homes. Remote risk of physical attack by hostile client.
REQUIRED KNOWLEDGE, SKILLS AND ABILITIES:
Minimum Training and Experience Required to Perform Essential Job Functions:
a. Bachelor’s Degree in Social Work and WI Social Work Certification or Bachelor’s Degree in a Human Service related field with current WI SW certification, or commitment to enroll in an accredited educational program within the first three months of employment and obtain Social Work Certification upon completion..
b. Three years’ experience with individuals or families with mental health, substance abuse, and/or abuse and neglect issues
c. Good written and oral communication skills; tact and courtesy; thoroughness and dependability
d. Knowledge of the functions, procedures, organization, policies, and regulations governing the administration of the Comprehensive Community Services Program
e. Ability to maintain confidentiality in dealing with all pertinent information
f. Ability to establish and maintain effective working relationships with peers and supervisors
g. Valid motor vehicle operator’s license and/or available means of transportation required.
h. Available to be on call as assigned
i. Meets requirements of background check including caregiver
