Duties

- Facilitate communication between clients, families, and service providers to ensure cohesive care.
- Conduct regular assessments to identify the needs of clients and recommend appropriate interventions.
- Ensure compliance with HIPAA regulations and maintain confidentiality of client information.
- Assist in conflict management situations to promote a safe environment for all clients.
- Support clients in daily living activities, including home care tasks as needed.
- Participate in case management meetings to coordinate services and resources for clients.
