Responsibilities:
Maintain regular ongoing communications and health coaching with members/enrollees regarding promoting, adopting, and maintaining healthy behaviors and wellness initiatives provided as part of AmeriHealth Caritas Wellness and Health Promotion programs. The role partners with members/enrollees by utilizing inclusive and person-centered motivational interviewing, active listening, and goal setting to develop member/enrollee participation and support in disease prevention and wellness promotion programs such as, but not limited to, tobacco cessation, weight management, stress management, nutrition counseling, sexual health counseling, environmental counseling, driver of health screening and support. This role supports integrated, holistic, person-centered programs such as the caring cards zero suicide concept encouraging/supportive phrases and invitations for outreach to help connect members/enrollees for follow-up support post-suicide attempt. Additionally, it supports post-NARCAN prescription fills. This role is pivotal to providing members/enrollees with education and linkage connection to local wellness center programs and resources as available. This role is part of the population health continuum of services and supports the coordination and triage of members/enrollees to other Population Health Programs and initiatives. This role works within the population health continuum to appropriately direct any clinical issues identified beyond the scope to appropriate escalation pathways via the Wellness Coordination Care Managers and/or the Market Population Health Clinical Coordination and Complex Care Management programs or external delegated care management entities.
Education/ Experience:
- Associate’s Degree required.
- Bilingual required.
- Licensed Practical Nurse preferred.
- Whole Health Action Management (WHAM) certification.
- Certified in Intrinsic Coaching (CIC).
- NCCA-accredited certification in fitness, nutrition, healthcare, wellness, human resources, or a related field from the Institute for Integrative Nutrition (IIN), National Board for Health and Wellness Coaching (NBHWC), American Council on Exercise (ACE), Health Coach Institute (HCI).
- 3 to 5 years Experience in wellness initiatives and the understanding, development, and participation in these initiatives.
- Experience as a health coach.
