Position Overview
We are seeking a dedicated Case Manager to provide comprehensive case management and support
services as an integral member of our treatment team in [facility location]. This role involves coordinating

Required Qualifications
Bachelor's degree preferred in Human or Social Services Field
OR Associate's degree with 2 years of relevant experience

Must be 21 years of age or older
Strong understanding of clinical treatment and social casework principles
Knowledge of child/adolescent growth and development
Experience working with children and adolescents with emotional and behavioral issues
Core Responsibilities
Clinical Coordination
Collaborate with therapists, resident care coordinators, and doctors
Coordinate documentation, discharge planning, and therapeutic passes
Participate in treatment planning meetings and program staff meetings
Contribute to quality management initiatives
Maintain accurate and timely client records
Family and Stakeholder Communication
Communicate effectively with families regarding therapeutic passes
Liaise with referral sources and mental health professionals
Provide detailed information about procedures and treatments
Integrate pass effectiveness information into treatment planning
Maintain confidentiality and privacy standards
Patient Care
Identify and address resident needs (social, legal, family, educational)
Respond appropriately to developmental age requirements
Demonstrate cultural and religious sensitivity
Recognize and report suspected abuse/neglect
Explain procedures in accessible language
Follow infection control protocols
Professional Conduct
Maintain a positive work atmosphere
Promote cooperation and teamwork
Adhere to organizational policies and code of ethics
Follow safety practices and report concerns promptly
Perform other related duties as assigned
