Role Responsibilities
Must-Haves:

- Lead efforts to improve care coordination across the healthcare continuum.
Must-Haves:
- Associate's Degree in Nursing
- Current RN license in NC or multi-state (compact) license
- BLS Certification
- CCM Certification (within first year)
- Minimum 3 years RN or Care Coordination experience (acute care)
- Bachelor’s or Master’s Degree in a related field
- ACMA Certification
- Minimum 3 years management experience
