Perry Creek Health Care & Rehabilitation Center is seeking a caring, warm-hearted Physical Therapist who is currently licensed in North Carolina or is a graduate from an approved Physical Therapist program and is actively working toward obtaining licensure. The Physical Therapist evaluates residents, develops effective treatment plans, and obtains approval for service from the referring physician. Treats residents according to the physician’s treatment plan. Participates in discharge planning. Records evaluation, treatment, and charges. Supervises Physical Therapist Assistants and Aides. Screens residents for potential benefit from PT intervention.
Requirements :
- North Carolina Physical Therapy License
- Demonstrates excellent oral and written communication skills.
- Demonstrates effective problem-solving skills.

PHYSICAL THERAPIST
MUST BE A GRADUATE OF AN ACCREDITED PROGRAM AS A PHYSICAL THERAPIST PT, HAVE TAKEN THE NPTE OR ELIGIBLE TO TAKE IT AND HAVE A STATE LICENSE TO PRACTICE AS ONE OR ELIGIBLE TO APPLY FOR ONE.
Duties / Responsibilities : Assesses the needs of residents with physical therapy needs and matches their needs with services available. Meets with residents regularly according to treatment program schedule, evaluates, and maintains records of residents' progress.
Reports progress to management and family members and meets with them on a regular basis to discuss.
