What are the responsibilities and job description for the Case Manager- San Antonio 2-2025 position at PerryLee Home Health Care Services, Inc?
Looking for full time case manager m-f 40 hours per week to work with a caseload of individuals receiving long term care waiver services. Responsibilities include coordinating, communication, documentation, develop service plans, calculate budgets, follow policy standards, conduct visits, and maintain current compliance overall in client chart.
Ideal candidate must have 2 years case management experience and working with physical or developmental disabilities is a plus. Bachelor’s degree in a related field is preferred but not required if experienced.
