Full Time, Part Time & Contractor Positions
The Case Manager will manage cases for their assigned clients to ensure that each client receives services needed for overall wellbeing and in accordance with the client's Individualized Treatment Plan (ITP).
RESPONSIBILITIES:
1. Utilize basic clinical interventions to support client efforts toward sobriety, coping with symptoms of depression and anxiety, safe lifestyle, healthy socialization, suicide prevention, occupational and academic success, individual and family restoration, and overall wellbeing.
2. Acquire knowledge and maintain current contact and referral information of community resources to address client needs for support of sobriety and mental stability, food, clothing, housing, financial assistance, inpatient referrals, medical care, and safety.
3. Complete and submit progress notes and other documentation within 24 to 48 hours.
4. Achieve or exceed individual productivity goals.
5. Review case files regularly to ensure on-going audit-ready status.
6. Collaborate with referral sources, local agencies, and legal entities as needed to coordinate client care, facilitate referrals, achieve, and maintain safety standards, promote good will, and expand opportunities for client resources and clinician training.
7. Acquire and maintain effective techniques for crisis de-escalation, as well as basic intervention for medical and mental health emergencies for use with clients in multiple settings.
8. Perform all job duties in a manner that is friendly, respectful, and aligned with the
agency's Mission, Values, Patient Care Philosophy, and Standards of Ethical Conduct.
9. All other duties as assigned.
QUALIFICATIONS:
1. High School Diploma or equivalent.
2. 3 years of experience working in the social work field.
3. Chemical Dependency certification or Social Work licensure, preferred.
4. Must be certified within 90 days of starting the position on First Aid, CPR, and De-escalation Training.
5. Be at least 23 years of age, have valid driver's license, and have an acceptable driving record.
6. Maintain a current and valid Ohio or Michigan driver's license, a good driving record, adequate insurance and be willing to use personal car to perform job duties.
7. Computer skills including Excel capabilities with ability to create reports.
