Registered Nurse (RN) –Home Health

Registered Nurse (RN) - Full-time, Part-time, Per Diem, and flexible scheduling options available!

Responsibilities:
- Provide skilled nursing visits to patients at their home setting.
- Assess and help establish patient treatment plans.
- Help develop, coordinate and implement the Nursing Care Plan working with the multi- disciplinary team.
- Provide skilled nursing care as ordered by the physician and outlined in the Care Plan.
- Monitor and report changes in patient symptoms or behavior.
- Communicate with physicians, other healthcare providers, and the careteam regarding patient care.
- Educate patients about health maintenance and disease prevention.
- Maintain timely and accurate patient medical records.
- Provide advice and emotional support to patients and their family members.
- Participate in on-call responsibilities as required (minimum one day per week and one weekend per month)
Requirements:
- Must be currently licensed in Pennsylvania as an RN
- Minimum one year home health and OASIS experience preferred.
- Reliable with good communication, organizational, and clinical skills.
- Have a caring, compassionate, personable and collaborative attitude.
- Familiarity with medical software and equipment.
- A valid driver’s license and reliable mode of transportation is required.

We provide a thorough training program focused on understanding home health rules and regulations, documentation requirements, application of clinical skills, and other necessary functions of the home health nurse. You can also expect to provide a variety of care in the patient's home, including but not limited to various types of wound care, administration of IV therapy, maintenance of central venous access devices including PICC lines and portacaths, ostomy care and maintenance, tube feeding and education, and other various patient education focused on disease process, medication regime, and identifying care-related issues and/or complications.
