JOB SUMMARY :   We are seeking a driven and relationship-focused  Home Care Marketing & Sales Liaison   (CHHA) to facilitate the transition of appropriate patients from our Licensed Home Care Services Agency (LHCSA) to our Certified Home Health Agency (CHHA) . This individual will play a vital role in identifying patients receiving personal care services who may benefit from skilled home health services, ensuring a seamless referral process and continuity of care.

QUALIFICATIONS :

Education : Bachelor’s degree in Marketing, Healthcare Administration, Business, or a related field preferred. Equivalent work experience will be considered.

Experience :

o Minimum of 2 years of marketing, business development, or referral coordination experience in home health, home care, or healthcare services.

o Knowledge of LHCSA and CHHA operations, services, and eligibility requirements.

Skills :

o Excellent relationship-building, communication, and networking abilities.

o Strong organizational and follow-up skills to ensure successful patient transitions.

o Ability to work collaboratively across multiple teams and departments.

o Proficiency in Microsoft Office Suite (Word, Excel, PowerPoint) and CRM software.

Other : Valid driver’s license and ability to travel within the service area as needed.

Job Type :  Full-time

Pay :   $70,000.00-$80,000.00 per year
