JOB SUMMARY :
As a H ospice H ome Care Consultant you will lead business to business customer solutions through sales, implementation, account management and home health care consultant to support the achievement of organizational strategies, specifically targeted the Hospice community. Collaborate with referral sources to generate referrals to Personal-Touch Home Care. Ensures Personal-Touch meets the needs of the referral sources. Works under general direction.
RESPONSIBILITIES :
1. Builds new and expands current Personal-Touch relationships with business-to-business customers that result in win / win business growth opportunities and effectively cross sell (as appropriate) all Personal-Touch Programs (Hospice, CHHA, Private Pay) where applicable.
2. Provides day-to-day Hospice Home Health Care consulting services support through continual contact with facility staff and / or physician / physician office staffs.
3. Identifies opportunities and participates in strategy development of new and / or enhanced business development opportunities designed to meet / exceed sales targets and support customer needs.
4. Profiles referral sources and identifies key opportunities to partner with new and existing customers that meet mutual goals.
5. Develops annual account plans; reviews account plans throughout the year to monitor progress against goal and the execution of account strategies. Updates account profiles, plans as needed.
6. Prepares and delivers presentations for new and enhanced programs and services to facility staff; physician offices and staff; and oversees implementation activities linked to such programs and services.
7. Partner with facility staff and / or physicians or physician office staff to address and problem solve issues.
8. Communicates with Operational staff as necessary related to capacity constraints, complaint resolution, ability to accept cases, clinical initiatives, teamwork building, customer inquiries as required to hit organizational goals.
9. Complete and submit weekly log of activities used to reach target.
10. Participates in special projects and performs other duties as assigned.
QUALIFICATIONS :
- Bachelor’s degree in health care administration, marketing, nursing or related discipline or equivalent work experience required.
- Minimum of 2 years of management, marketing or sales experience required, preferably in hospice, health care, nursing, social services, or related discipline.
- Ability to negotiate to win-win outcomes and demonstrate exceptional customer service skills.
- Strong detail-oriented and follow up skills, as well as the ability to manage multiple priorities.
- Knowledge of health plans, Medicare, Medicaid, related to overall home health benefits.
