Job description :
Intake Referral Coordinator - LPN
Want a change of pace and scenery? Looking to continue utilizing your LPN skills but need a break from hospital / facility work? Bring your LPN skills and experience to Personal Touch and work in the comfort of an in-office work environment with an 8 : 30 to 4 : 30 schedule - Monday - Friday. Keep your weekends FREE or pick up LPN visits if you want to!
Prefer candidate to hold an active LPN license in the State of Virginia. Position is located in our Newport News office. The role assumes the day-to-day responsibility for ensuring superior customer service by coordinating patient referrals for home care services. Ideal candidate will have prior home health care experience and / or Intake experience. Responsible for the smooth flow and continuity of patient care within the department. This position offers a great team-oriented office work environment!
Essential JOB FUNCTIONS :
MUST present a positive and upbeat phone voice to all incoming calls as this position is the first impression to our referral sources and or patients / family members.
Answers phones for Intake information and enters into the EMR system.
Completes intake form, ICD-10 Coding and routes it appropriately for admission approval. Forwards the intake to PBR for insurance verification / credit approval / Medicare number verification.
Conducts initial nursing insurance authorization on new referrals and as needed, maintaining a tracking log for insurance authorizations and entering into the EMR system.
Maintains referral log for all intakes weekly.
Communicates hard to staff cases to the Intake Manager for further review and consideration.
Assists with in-servicing clinical staff when an educational opportunity presents.
Performs all applicable duties of an LPN in the event that visits are uncovered, and clinical assistance is needed.
Works to develop a positive rapport with all staff members and community resources receiving home care services.
Assists in other branches as needed.
