Personalized Nursing LIGHT House is now hiring several SUD Nurses that will provide support for residents in treatment for PNLH preference for candidates with training in, and experience with, providing trauma related services.“ We take affirmative efforts to hire individuals with disabilities, including those in recovery”.

We have locations in Plymouth, Canton, Madison Heights. Specifically, our intent is to increase client well-being, decrease addiction severity, improve the quality of clients' mental and physical heath and contribute positively to society. All services will integrate substance use disorder, mental health and primary health care treatment in cohesive and client-centered programs.
Responsibilities :
- Monitor the medical status of all persons served (via vitals, monitor medications, periodic observations of persons served, special attention to detox withdrawal symptoms).
- Provide medical and nutritional didactics / groups to the persons served.
- Conduct individual health assessments for each client that enters STR, with a nursing care plan developed (day shift LPN primary responsible).
- Observe & monitor the client’s vital signs, physical and emotional status using the CIWA assessment form.
- Facilitate didactic groups with clients related to drug use, recovery, and other relapse prevention strategies.

- Monitor client in self-administration of medications for relief of symptoms. Maintain accurate medication log.
- Engage client in conversations regarding treatment readiness, hopefulness, and strategies to achieve and maintain a clean, sober lifestyle.
- Network with and become familiar with other local agencies and services that might be of service to clients during and after social detox placement.
- Responsible to document all behaviors, physical symptoms, and observations of clients during shifts, according to clinical protocols, including critical incidents.
- Develop rapport and a professional therapeutic relationship with clients.
- Completes phone intakes, gathering all relevant information to ensure the client is eligible and appropriate for admission to the detox program.
- Facilitate client-centered, engaging intake process. Coordinate client’s arrival to detox program; provide program orientation, establish a therapeutic relationship, and gather initial information and paperwork from the client.
- Responsible for knowing and adhering to the health and safety policies. Assist in maintaining a clean, sanitary and safe environment for clients by taking the initiative in household cleaning, meal preparation / clean-up, laundry, and minor household repair.
- Ensure all essential detox / STR functions are maintained in the absence of other team members including, but not limited to, coordination of client placement, client meals, ensuring safe, clean environment, phone coverage, essential data entry, and basic housekeeping / laundry.
- Perform all other related and / or assigned tasks by management.
