Position Overview: We are seeking a highly organized and detail-oriented individual to join our team as a Patient Services Coordinator for our Mobile Wound care service. As a Patient Services Coordinator, you will play a crucial role in facilitating the smooth operation of our mobile wound care program by coordinating patient intake, scheduling appointments, and ensuring seamless communication between patients, healthcare providers, and our mobile wound care team. This position requires exceptional communication skills, strong attention to detail, and a compassionate approach to patient care.
Responsibilities:
Patient Intake Coordination:Receive and process patient referrals for mobile wound care services.
Collect and verify patient information, including medical history, contact details, and insurance details.
Ensure accurate and complete documentation of patient information in electronic health records (EHR) or other designated systems.
Collaborate with healthcare providers and insurance companies to obtain necessary authorizations and verify insurance coverage.
Appointment Scheduling and Coordination: Schedule patient appointments for mobile wound care services based on availability of the mobile wound care team and patient preferences.
Coordinate appointment logistics, including arranging transportation for patients as needed.
Communicate appointment details, including date, time, and location, to patients and healthcare providers.
Communication and Follow-up: Serve as the primary point of contact for patients, healthcare providers, and the mobile wound care team.
Provide clear and timely communication to patients regarding appointment reminders, rescheduling, and any necessary preparations.
Coordinate with healthcare providers to obtain and relay relevant patient medical information and wound care instructions to the mobile woundcare team.
Collaborate with the mobile woundcare team to ensure timely documentation of patient assessments, treatments, and outcomes.
Administrative Tasks: Maintain accurate and up-to-date records of patient interactions, appointments, and referrals.
Assist with billing and insurance-related tasks, including verification of coverage and submission of claims.
Contribute to quality improvement initiatives by identifying areas for process optimization and suggesting improvements.
Requirements:
- Bachelor's degree in healthcare administration, nursing, or a related field (preferred).
- Prior experience in a healthcare coordination or administrative role is highly desirable.
- Familiarity with medical terminology, wound care procedures, and EHR systems is advantageous.
- Excellent organizational and time management skills, with the ability to prioritize tasks effectively.
- Strong communication skills, both written and verbal, with the ability to interact compassionately and professionally with patients and healthcare providers.
- Proficiency in using computer systems and software applications for data entry and record keeping.
- Ability to work independently, demonstrate critical thinking, and problem-solve in a fast-paced environment.
- Knowledge of HIPAA regulations and maintaining patient confidentiality.
