Position Summary
The Medical Records and Coder position plays a vital role in ensuring the accurate and timely handling of medical records in compliance with the Health Insurance Portability and Accountability Act (HIPAA). This role involves managing the transmission and receipt of medical records, performing medical coding, and supporting the Health Information and Manager (HIM) department.
Key Responsibilities
Medical Records
Process medical record requests and coordinate with the HIM department as necessary.
Ensure all requests include a signed release of information form.
Handle requests from attorneys, insurance companies, disability claims, and state agencies, ensuring appropriate charges for copying and postage.
Manage both electronic and paper records, ensuring timely and accurate distribution.
Request patient records from outside facilities for continued patient care, following up as needed.
Assist in the secure destruction of paper records, ensure proper documentation.
Maintain confidentiality and compliance with HIPAA regulations.
Provide backup support for the Clinic Medical Records as needed.
Medical Coding
Apply CPT and ICD-10 coding guidelines for accurate medical record coding.
Identify and correct coding discrepancies.
Communicate with internal teams, including registration, providers, nursing, lab, radiology, and medical assistants, to resolve discrepancies.
Chart Review and Filing
Ensure compliance with HIPAA and HITECH laws.
Review and process medical records requests from patients, healthcare facilities, legal entities, insurance companies, and the Social Security Administration.
Handle requests via mail, fax, encrypted email, patient portals, and in person.
Track disclosures of medical records using a logging system.
Submit charge sheets for record requests to the billing office.
Request patient records from other healthcare facilities as needed.
Maintain file organization, including thinning and shredding inactive or deceased patient charts according to proper procedure.
Additional Responsibilities
Provide support for other HIM team members as needed.
Perform other duties as assigned.
Qualifications
Associate degree or minimum of two (2) years' experience in medical coding and record experience in healthcare setting preferred but not required.
Computer experience is helpful, especially MS Excel, MS Word MS Outlook. Type minimum of 40 wpm preferred.
Strong organizational skills, high attention to detail, and ability to prioritize tasks in a fast-paced, multi-departmental environment.
Must possess the ability to prioritize responsibilities and handle multiple requests simultaneously.
Strong work ethic and dependability, and the ability to handle confidential information appropriately.
Must be able to pass a State of Alaska background check and a drug screen (including Marijuana).
