This position is PRN, number of shifts may vary throughout the week. 8am-5pm

What you will do :
The Home Care LVN is a professional nurse who collaborates with the provider and supervising RN for the home care patient's care using focused assessment skills, teaching and education abilities in their home environment (private residence, assisted living, etc.) based on individual needs. The Home Care LVN provides patient care based on supervising RNs delegation, provider's orders, departmental policies and procedures, regulatory guidelines and best practice. The Home Care LVN documents in the patient's medical record in a timely manner according to agency policy. The Home Care LVN attends and actively participates in Case Conference and team meetings. The Home Care LVN performs on-call responsibilities and Holiday staffing.
Required Education :
Current Licensed Vocational Nursing Degree
Required Skills :
Nursing skills as defined by generally accepted standards of practice. Basic knowledge and skills to care for the clinical needs of the population served. Professional and efficient verbal and written communication skills. Professional interpersonal skills. Ability to multi-task, basic organizational and time management skills. Basic computer skills. Knowledge of general office and administrative practices and procedures. Knowledge of how to utilize office equipment (i.e., copy machine, fax machine, etc.) Must have demonstrated experience in working with patients and caregivers dealing with acute and chronic illness. Must be capable of prioritizing and handling multiple tasks of critical importance simultaneously.
Preferred Skills :
Experience with Home Care Home Base; Experience with smart phones and smart devices
Required Licensure / Registration / Certification :
Current Texas state license in appropriate discipline; Current American Heart Association CPR; Current Texas state driver's license; Current auto insurance. Must have reliable transportation.
Preferred Licensure / Registration / Certification :
Wound Care certified; Advanced education or certification in area of specialty
Required Experience :
One year experience in a healthcare setting in respective discipline
Preferred Experience :
Medical-Surgical experience; Experience in Home Care or Hospice
