What you will do:
The RN Case Manager, in collaboration with the clinical team, provides discharge planning coordination and intervention. The RN Case Manager will perform concurrent and retrospective review of medical records for purposes of utilization management and compliance with requirements of external review agencies including governmental and non-governmental payers. The RN Case Manager strives to promote patient wellness, improved patient outcomes, efficient utilization of health services and minimize denials payment.

Required Education:
Graduation from an accredited School o Nursing

Preferred Education:
Bachelor of Science in nursing

Required Skills:
Basic computer and keyboarding

Preferred Skills:
Meditech Electronic Medical Record.

Required Experience:
Three years acute care nursing experience

Preferred Experience:
Case Management experience
