SUMMARY of DUTIES:

INTAKE:

  • Process client referrals received from Regional Offices and other TCM entities by managing,
  • staying current with regulations for, and implementing the intake process for
  • MPower TCM. Serves as the point of
  • contact for the agency for incoming clients.
  • Assess intake referrals to ensure that the referral to MPower is appropriate and MPower
  • can serve the individual referred.
  • Coordinate and communicate with SC Supervisor and SC about the eventual assignment of the
  • client to a caseload.
  • Enter demographic information into SET-works.
  • Create Designated Record Set (DRS) for client.
  • Confirm the MPower acceptance date to the sending TCM/RO/internal staff.
  • Receive file from sending entity and assure files are placed in MPower DRS and named
  • according to the DRS File Format.
  • Utilize MPower TCM file checklist to assure all required TCM docs have been received.
  • Track intake information for data collection purposes.


INITIAL INTAKE SERVICE COORDINATOR ROLE & RESPONSIBILITY

  • Alongside a newly referred client and their team, develop an initial Individual Support Plan (ISP). This plan will include all required
  • information from the Department of Mental Health (DMH) and MPower. The plan will clearly identify the reason for
  • the client to enter MPower services, the needs and hopes that have been
  • communicated by the client and their team, and identify goals and action plans
  • for the permanent Service Coordinator to follow up on.
  • When applicable, make an initial referral for CMAP services to Easter Seals Midwest (ESMW).
  • Once an initial ISP is finalized (and ESMW has agreed to serve the client, as applicable), transition client to their
  • permanent Service Coordinator by coordinating with the county-specific
  • Supervisor.
  • Complete the New Client Information coversheet to transition client to permanent Service Coordinator.


TRANSFER INTAKE SERVICE COORDINATOR ROLE & RESPONSIBILITY

  • May coordinate information between sending TCM/RO and MPower to assure compliance
  • with transition & transfer directives/processes.
  • May attend transition meeting(s) for incoming clients to ensure tasks associated
  • with transition/transfer are completed including reviewing the Residential
  • Transition Checklist (if applicable).
  • Ensure the checklist is filed in the MPower DRS.
  • May be asked to complete courtesy monitoring on behalf of sending TCM.
  • Will review the current plan to assess if it reflects information on why the client
  • is now served by MPower.
  • May have to complete a site review if this is for a new residential home in our
  • area. Ensure this site review gets
  • placed in the record set.
  • Confirms acceptance of transfer with sending TCM/RO.
  • Stays in close communication with permanent MPower SC.
  • May make efforts to track down missing files from sending TCM/RO. This may be completed in conjunction with the
  • Manager.
  • May be asked to complete Missing Documents Memo for missing files.


DISCHARGE:

  • Process client discharges and transfers from by managing, staying current with
  • regulations for, and implementing the discharge process for MPower TCM. Serves as the point of contact for the agency
  • for outgoing clients.
  • Process transfer request packet from SC and coordinate with receiving TCM and/or RO
  • staff for transfer acceptance.
  • Coordinate information between sending and receiving TCM and/or RO to assure compliance
  • with transition & transfer directives/processes.
  • Communicate with RO & MPower Management any concerns as needed.
  • Confirm acceptance of transfer with receiving TCM and/or RO and notify SC to proceed
  • with completing Discharge Summary & COS.
  • Notify Admin Asst to update CIMOR and SET-works.
  • Process Discharge Summary & COS from SC and places in DRS.
  • Collect and places final case notes in DRS.
  • Assure MPower DRS is uploaded to FTP server at the appropriate time once receiving TCM
  • accepts transfer or file is needed by the RO.
  • Move active DRS to inactive DRS at appropriate time once discharge is finalized by
  • MPower TCM
  • Communicate discharge date with Admin Asst, SC Supervisor, Manager, & receiving TCM
  • and/or RO
  • Track discharge information for data collection purposes.


TRAINING

  • Responsible for implementing the New Hire Training and Exposure program in coordination
  • with Supervisors. This may include but
  • is not limited to:
  • Setting New Hire up with the Relias and MELS training.
  • Serving as the first point of contact for setting up training opportunities and
  • questions regarding training courses and related material.
  • Training & exposing New Hire to tasks identified on the Training & Exposure Log
  • in coordination with Supervisors.
  • Coordinating shadowing opportunities for the New Hire with other Service Coordinators to
  • meet quota.
  • Arranging observation sessions with applicable Supervisor and New Hire
  • Ensure new hire stays on track to complete the New Hire Training & Exposure Log
  • tasks.
  • In consultation with Supervisors and Manager, will be responsible for keeping the
  • New Hire training program current and applicable using directives/guidance from
  • Department of Mental Health and MPower leadership.
  • May be asked to attend webinars and other trainings that DMH makes available
  • regarding new or adjusted processes.
  • May be asked to collaborate with MPower leadership regarding how a process will be
  • developed and communicated.

CASELOAD

May provide professional Service Coordination services to eligible individuals by performing the following duties in an environment that promotes the individual served:

Applies professional judgment and discretion in determining and securing Medicaid and non-Medicaid habilitative services to ensure both active treatment and maintenance of skills aimed towards supporting individuals served to progress towards their identified goals and needs for

Supports and/or refers individuals served to navigate complex requirements and regulations to receive appropriate services.

Manages a caseload of minimal size and complexity, normally involving a wide range of disability areas and age ranges.

o Managing a caseload may include administrative duties such as tracking, mailings and utilization review functions.

Secures information such as medical, psychological, and social factors contributing to individual's situation to assist in completing the Individual Support Plan (ISP); secures supplementary information such as employment, medical and/or school records. The ISP is written by the Service Coordinator in conjunction with the individual, family, guardian, providers, etc. according to MPower and Department of Mental Health Division of Developmental Disabilities (DMH-DDD) guidelines.


Maintains a working knowledge of and locates, refers, arranges, and accesses community resources available for the individual served. Reviews both formal and informal assessments of individuals served for determining eligibility for Medicaid Waiver funding. Staff will be trained/certified upon hire, as appropriate.

Develops and maintains records regarding services rendered and prepares reports concerning the individual served and progress toward ISP outcomes.

Facilitates team meetings to discuss progress and possible modifications to services in the ISP; completes addendums as needed.

Makes contact with each individual served and completes monitoring of the ISP as required by MPower policy and DMH-DDD directives.

Serves as a liaison between multiple agencies and individuals served to ensure that the individual's needs are being addressed.

Uses professional judgement to communicate and receive assistance from Supervisor and/or SC Lead as needed.

GENERAL EXPECTATIONS, SKILLS, and ABILITIES

  • Attends and participates in staff meetings, in-services and staff development training.
  • Utilizes Worksight time keeping system to request time off. Mileage should be reflected in Worksight for the day it was used within the pay period. As a salaried employee, work hours may vary based on workload demands and deadlines. Employees in this position are expected to manage their time effectively to fulfill job responsibilities.
  • Accepts and willingly carries out special assignments or duties when requested.
  • Projects a positive image of MPower and individuals served to the local community.
  • Assures confidentially of information regarding individuals served.
  • Respects the rights of all individuals served and agency staff.
  • Utilizes personal vehicle for the completion of work duties as needed. This includes, but is not limited to, driving to appointments, trainings, and meetings while on the clock.
  • Keeps current schedule updated as reflected in Microsoft Outlook.
  • Communicates with the Service Coordination team in a manner that provides a positive agency culture and adds value to the mission and vision of MPower.
  • Replies to all correspondence and phone messages within 48 hours.
  • Will be required to use the following programs/devices: CIMOR, SET-works, Microsoft programs andapplications, a computer, Worksight, Bamboo HR, G&A portal, Adobe, PDF ProConverter, and other agency and State of Missouri programs/databases.
