The Chronic Care Manager is responsible for gathering and organizing patient data, identifying unmet needs, engaging patients in their care, and enhancing ongoing communication between the patient and their care team. This position requires excellent problem-solving skills, effective communication, and the ability to multitask.
- Develop a keen understanding of primary care practice requirements for optimal, coordinated population health.
