Job Summary
We are seeking a highly skilled Pharmacist to join our team. In this role, you will be responsible for evaluating physician medication orders, maintaining patient/resident databases, and collaborating with healthcare professionals to ensure high-quality patient care.
Responsibilities:
- Evaluate physician medication orders for appropriateness, potential interactions, and route of administration.
- Maintain patient/resident databases, including current and past medical history, clinical and laboratory data, and concurrent medication.
- Ensure that appropriate laboratory tests are ordered, evaluated, and discussed with healthcare team members.
Requirements:
To be considered for this role, you must have a Bachelor's Degree in Pharmacy, Pharm D preferred, and one year of experience in long-term care. You must also possess excellent communication and interpersonal skills, as well as the ability to work effectively in a fast-paced environment.
