Job Summary
We are seeking a highly skilled Medication Management Pharmacist to join our team. In this role, you will be responsible for evaluating physician medication orders, making recommendations for therapy changes, and maintaining patient/resident databases.
You will also collaborate with healthcare teams to provide clinical and technical support, document communication with physicians and patients, and implement policies and procedures for safe medication procurement and use.

Key Responsibilities
- Evaluate physician medication orders for appropriateness, potential interactions, and route of administration
- Make recommendations for therapy changes to physicians and facility staff as appropriate
- Maintain accurate and up-to-date patient/resident databases, including current and past medical history, clinical and laboratory data, and concurrent medications
- Collaborate with healthcare teams to provide clinical and technical support, as needed
- Document all communication with physicians, nurses, and patients/residents and their family members in the medical record
- Implement policies and procedures for safe medication procurement, storage, distribution, and use
- Inform the Pharmacy Manager of medication-related errors
- Assist facility/agency staff personnel with procedures relating to safe and effective methods of procuring, maintaining, administering, and disposing of drugs
Requirements
To be considered for this role, you must have:
- A Bachelor's Degree in Pharmacy; Pharm D preferred
- One year of experience in long-term care
- Licenses/Certifications: As required by government regulations and business needs
- Demonstrated ability to work effectively in a fast-paced environment and prioritize tasks to meet deadlines
- Excellent communication, interpersonal, and problem-solving skills
