Location: 100% Remote (Las Vegas Resident)
Job Summary:We are seeking a compassionate and detail-oriented Patient Care Manager to lead our Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) programs. This role focuses on empowering patients to manage their chronic diseases effectively while ensuring high-quality care delivery. The ideal candidate will conduct comprehensive assessments, develop individualized care plans, and provide patient-centered education and support. Candidates must be certified Medical Assistants (MA) or Licensed Practical Nurses (LPN) with a strong background in chronic disease management.
Key Responsibilities:
- Comprehensive Patient Assessments:
- Conduct thorough assessments of patients' medical, psychological, and social needs.
- Identify barriers to effective disease management and collaborate with patients to address them.
- Individualized Care Plans:
- Develop and implement personalized care plans in collaboration with a multidisciplinary team, including physicians, pharmacists, and other healthcare professionals.
- Regularly review and adjust care plans based on patient progress and changing needs.
- Patient Education and Engagement:
- Educate patients on managing chronic diseases, including medication adherence, vital sign monitoring, nutrition, exercise, and lifestyle modifications.
- Utilize motivational interviewing and behavior-change strategies to help patients achieve their health goals.
- Care Coordination:
- Facilitate communication between patients, caregivers, and healthcare team members to ensure seamless coordination of care.
- Follow up on referrals, test results, and other care needs to prevent gaps in treatment.
- Data Monitoring and Reporting:
- Monitor RPM data for trends or potential health risks and escalate findings to the appropriate care team members.
- Use data insights to guide patient interventions and improve outcomes.
- Documentation and Compliance:
- Document patient interactions, care activities, and assessments accurately in the electronic health record (EHR) system.
- Ensure compliance with CMS requirements for CCM and RPM programs.
- Program Management:
- Assist in enrolling eligible patients into CCM and RPM programs.
- Monitor program effectiveness and patient satisfaction to identify opportunities for improvement.
Qualifications and Requirements:
Education & Certification:
Certified Medical Assistant (MA) or Licensed Practical Nurse (LPN) credential required.
Certification in disease management, case management, or patient education is a plus.
Experience:
- At least 2 years of experience in patient care, care coordination, or chronic disease management.
- Experience with CCM or RPM programs strongly preferred.
Technical Skills:
- Proficiency in electronic health records (EHR) systems.
- Experience with Google Workspace
- Familiarity with RPM devices and data monitoring tools is an advantage.
Soft Skills:
- Excellent interpersonal and communication skills for patient education and team collaboration.
- Strong organizational and multitasking abilities with attention to detail.

Pay: $35,161.00 - $69,749.00 per year
