Key Responsibilities :
- Conduct comprehensive assessments to determine patient needs and develop individualized treatment plans
- Implement and deliver rehabilitation therapy to patients in the comfort of their homes or care facilities
- Continuously evaluate patient progress and adjust care plans as needed
- Collaborate with other healthcare professionals to ensure the best patient outcomes
